About The Position

Unicity Care Management supports individuals and families through significant healthcare decisions, including new diagnoses, complicated hospital discharges, and chronic conditions that families struggle to coordinate. The Medical Social Work Care Manager and Patient Advocate serves as a primary contact for clients and families, providing care management and advocacy to a deliberately small private-pay caseload. The role offers autonomy to exercise professional judgment and build a schedule around client visits. It is field-based, requires regular travel, and is not an in-office position. The position also provides high-touch, concierge-level support, including accompanying clients to medical appointments and other care-related visits as needed. The role is hybrid, with weekday work and flexibility in how the workweek is arranged. The care manager may need to respond to urgent client needs outside standard business hours.
