About The Position

Unicity Care Management is hiring a Medical Social Work Care Manager and Patient Advocate to support individuals and families navigating healthcare decisions, new diagnoses, hospital discharges, and chronic conditions. The role carries a deliberately small private-pay caseload, allowing the care manager to build relationships with clients and families, exercise professional judgment, and arrange a flexible schedule around client visits. The position is field-based, requires regular travel, and is not an in-office role. It provides high-touch, concierge-level support, including accompanying clients to medical appointments and other care-related visits as needed. The care manager serves as the primary contact for their clients, including responding to urgent needs outside standard business hours. The position offers a base salary of $70,000–$90,000, plus a monthly performance bonus based on billable hours. Benefits include company-sponsored health, dental, and vision coverage; a safe harbor 401(k) with company contributions fully vested from day one; voluntary life insurance, short-term disability, and AD&D coverage; 15+ PTO days and paid holidays; a company phone and laptop; and reimbursement for mileage and tolls. The role is described as hybrid, weekday-based, and flexible, while also requiring regular field travel.
