Medical Director/Attending Physician: Post-Acute & SNF (Part-Time)
About The Position
Theoria Medical is seeking a part-time Medical Director and Attending Physician to serve local skilled nursing facilities. The role emphasizes continuity of care and building lasting relationships with residents in a collaborative, mission-driven clinical community. Physicians have autonomy over their schedules and support from a clinical team, with technology intended to streamline documentation and care delivery. Theoria Medical provides multispecialty physician services and technology to skilled nursing facilities. Physicians cover each facility one to two days per week, with no fixed start or end times and no on-call or overnight requirements. Compensation is productivity-based, with annual performance and longevity bonus opportunities. The estimated compensation range is $86,000 to $194,000 per year, based on expected earnings.
Requirements
- Physicians must hold an MD or DO degree.
- Must hold an active U.S. medical license and be board-certified in an appropriate specialty.
- Board-eligible new graduates actively pursuing licensing are encouraged to apply.
- Qualified specialties include Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.
- Must be able to perform the essential functions of the position satisfactorily, with or without reasonable accommodation.

Responsibilities
- Serve as the attending physician for patients in assigned skilled nursing facilities.
- Collaborate with NPs/PAs to manage daily patient care and provide clinical supervision.
- Provide patient-centered primary care, including diagnosing and treating acute and chronic conditions.
- Participate in Chronic Care Management, Transitional Care, Remote Patient Monitoring, and other quality initiatives.
