The Los Angeles County Department of Homeless Services and Housing (HSH) consolidates the countywide response to homelessness, aiming to increase accountability and transparency, improve care for those experiencing or at risk of homelessness, and streamline collaboration with partners. The Homeless Prevention Unit (HPU) is a collaborative effort between HSH, California Policy Lab (CPL), and the Department of Mental Health (DMH). HPU uses predictive analytics to identify individuals at risk of homelessness and provides early interventions to prevent housing loss. The unit is expanding to serve young adults aged 18-24 with proactive homelessness prevention resources. The Medical Case Worker II (TAY Case Manager) will serve as a case manager for the HPU, reporting to the TAY Program Manager. This role involves engaging individuals at risk of homelessness, implementing interventions, and making linkages to maintain housing stability. Staff schedules are based on business need and may include a hybrid work schedule.
