About The Position

Countryside Post Acute is hiring a full-time, onsite MDS-RN in Buchanan, Georgia. The role focuses on assessing residents, developing personalized care plans, and collaborating with interdisciplinary teams to support high-quality, patient-centered care and improve resident outcomes. The MDS-RN documents assessments and care, maintains compliance with state and federal regulations, communicates with residents, families, and staff, participates in care conferences, and contributes to Quality Improvement initiatives. The position also involves staying current with industry best practices and supporting continuous improvement.

Responsibilities

Conduct comprehensive resident assessments and develop personalized care plans.

Participate in care conferences.
