About The Position

Priority Management Group is seeking an MDS Nurse Coordinator (LPN or RN) for Mansfield Medical Lodge in Mansfield, Texas. The position supports the organization’s care teams, which provide dedicated and compassionate care to patients and residents. The MDS Nurse Coordinator is responsible for timely and accurate completion of the Resident Assessment Instrument (RAI) and care management processes from admission through discharge, following company policies and procedures and applicable federal, state, certification, and other relevant guidelines. Under the direction of the VP of Clinical Reimbursement, the coordinator may also coordinate clinical department information-systems operations and education.

Requirements

Current, unencumbered, active LPN/LVN or RN license in the state.
Training in rehabilitative and restorative nursing practices.
Working knowledge of managed care rules, regulations, and guidelines.
Ability to plan, organize, develop, implement, and interpret programs, goals, objectives, policies, and procedures necessary to provide quality care.

Nice To Haves

Three years of MDS experience in a nursing care facility preferred.

Responsibilities

Complete the RAI and care management processes accurately and on time from admission through discharge.
Complete Minimum Data Set, discharge, and admission tracking requirements in accordance with company policy and applicable guidelines.
Under the direction of the VP of Clinical Reimbursement, coordinate information-systems operations and education for the clinical department as needed.
