About The Position

Mozaic Senior Life is seeking an experienced MDS Coordinator for a full-time, 40-hour-per-week position. The coordinator is responsible for all MDS assessments in long-term care, including initial admission, quarterly, change-of-condition, Medicare, and managed Medicare assessments. The role works collaboratively with nurse managers and the interdisciplinary team (IDT) to ensure care plans reflect residents’ conditions on an ongoing basis and ensures that all Medicare residents are skilled.

Requirements

• Graduate of a school of professional nursing.
• Strong clinical background in nursing.
• Registered Nurse licensure in the State of Connecticut.
• CPR certification.
• IV-line management 3.0 certification.

Nice To Haves

• Bachelor of Science in Nursing (BSN) preferred.
• Head nurse experience.
• Care planning experience.
• MDS experience.

Responsibilities
