MDS Coordinator (LPN)
About The Position
Signature HealthCARE at Heritage Hall Rehab & Wellness Center is a 112-bed facility offering services from short-term rehabilitation to traditional long-term care. Signature HealthCARE is a family-based healthcare company providing integrated services across multiple states, including skilled nursing, rehabilitation, assisted and memory care, and home-based services supported by technologies such as telehealth and Care.ai-enabled solutions. The MDS Coordinator gathers information for resident assessments, participates in meetings related to Medicare and federal billing requirements, and reviews plans of care. The ideal candidate is detail-oriented, organized, and self-motivated, while also conducting bedside resident assessments and meeting with families. The position offers no call rotation or weekend requirement, and MDS growth tracks are available.
Requirements
- Current state licensure as a Licensed Practical/Vocational Nurse.
- At least three years of clinical experience in a healthcare setting; long-term care experience is preferred.
- At least one year of MDS experience.

Responsibilities
- Collect information to complete the MDS using medical records, bedside assessments, and interviews with staff, residents, and/or families.
- Participate in daily PPS meetings, weekly Medicare meetings, and month-end meetings to help ensure federal billing requirements are met.
- Review the Plan of Care at least quarterly and with each Comprehensive Assessment to ensure changes during the quarter are included and updated.
