About The Position

The MDS Nurse (LPN), under the direction of the Director of Nursing (DON), coordinates the Resident Assessment Instrument (RAI) process, including completion of an accurate Minimum Data Set (MDS) and collection of data to develop the Interdisciplinary Plan of Care. The position participates in the daily Medicare Prospective Pay System (PPS) Interdisciplinary Team Meeting and works as part of the nursing management team.

Requirements

Current, unencumbered LPN license, as required by state.
Ability to utilize local and consultant resources in carrying out job responsibilities.
Ability to develop adequate knowledge of state, federal, and regulatory requirements related to the job.
Reliable transportation required.

Nice To Haves

At least 1 year of direct hands-on nursing and MDS/RAP experience preferred.

Responsibilities

Coordinate the Resident Assessment Instrument (RAI) process.
Complete an accurate Minimum Data Set (MDS) and support development of the interdisciplinary Plan of Care.
Participate in the daily Medicare Prospective Pay System (PPS) Interdisciplinary Team Meeting (ITM).
Establish and maintain positive relationships with all members of the healthcare team.
Function as part of the nursing management team.
