About The Position

Avalon Care Center at Northpointe in Spokane, Washington is hiring an MDS Coordinator with an LPN license. This role is a key member of the clinical leadership team and combines clinical expertise with knowledge of the MDS process to support resident care and facility success. The position offers a $5,000 sign-on bonus and pays $50.00–$52.00 per hour, based on experience, qualifications, and skills. It is full-time, at one work location, with consistent scheduling and no nursing call responsibilities.

Requirements

• Current LPN license in Washington State or eligibility under the Nurse Licensure Compact (NLC).
• Previous MDS and skilled nursing facility experience.
• Knowledge of the RAI process and case management.
• Strong communication and organizational skills.
• Ability to work effectively within a team.
• Ability to pass employment screenings.

Responsibilities

• Coordinate and complete MDS assessments accurately and on time.
• Oversee Medicare, Managed Care, and OBRA assessment processes.
• Support care planning and interdisciplinary team collaboration.
• Help ensure regulatory compliance and reimbursement accuracy.
• Monitor resident assessments from admission through discharge.
• Promote quality outcomes and resident-centered care.
