About The Position

Care Initiatives is seeking a compassionate Licensed Practical Nurse (LPN) to work as an MDS Coordinator in one of its skilled nursing communities in Iowa. The role provides comprehensive care and supports residents and team members throughout the healthcare journey. The position offers an opportunity to apply nursing skills and compassion while contributing to residents’ well-being and supporting their loved ones and the care team.

Requirements

• Valid Licensed Practical Nurse (LPN) license in good standing, valid in Iowa or with the ability to activate it.
• Current, valid CPR certification.
• MDS credential, or willingness to obtain one.

Nice To Haves

• Familiarity with the MDS 3.0/RAI Manual.
• Work experience in long-term care and gerontological nursing.
