About The Position

Avalon Care Center at Northpointe is hiring a full-time MDS Coordinator (LPN) in Spokane, Washington. This role combines clinical expertise with knowledge of the MDS process and is a key member of the clinical leadership team. The coordinator supports resident care and facility success while building relationships with residents, families, and staff. Previous MDS and skilled nursing facility experience is required. Pay is $50.00–$52.00 per hour, based on experience, qualifications, and skills. A $5,000 sign-on bonus is offered.

Requirements

• Current LPN license in Washington State or eligibility under the Nurse Licensure Compact (NLC).
• Previous MDS and skilled nursing facility experience.
• Knowledge of the RAI process and case management.
• Strong communication and organizational skills.
• Ability to work effectively within a team.
• Ability to pass employment screenings.

Responsibilities

• Coordinate and complete MDS assessments accurately and on time.
• Oversee Medicare, Managed Care, and OBRA assessment processes.
• Support care planning and interdisciplinary team collaboration.
• Help ensure regulatory compliance and reimbursement accuracy.
• Monitor resident assessments from admission through discharge.
• Promote quality outcomes and resident-centered care.
