About The Position

Avalon Care Center Federal Way is seeking an experienced MDS Coordinator who is a Licensed Practical Nurse (LPN). This full-time role is a key financial and clinical member of the management team. The coordinator oversees services provided to Medicare and Managed Care residents during their inpatient stay, from admission through discharge, and ensures accurate, timely completion of required OBRA assessments. Requirements

Hold a current, active license to practice as a Licensed Practical Nurse (LPN) in the state of practice.
Be well-versed in interpersonal communications.
Be familiar with the RAI process and case management.
Be able to read, write, and speak English.
Meet applicable local health regulations.
Pass a post-employment physical exam if required, as well as drug screening, criminal background investigation, and reference inquiry.

Responsibilities

Oversee services provided to Medicare and Managed Care residents during their inpatient stay, from admission through discharge, as directed by the Resident Assessment Director.
Ensure accurate and timely completion of all required OBRA assessments.
