About The Position

Cura of Sauk Centre is seeking a detail-oriented and experienced MDS Coordinator, RN Manager. The role oversees Minimum Data Set (MDS) assessments to ensure accurate and timely documentation that supports quality care and regulatory compliance. The coordinator collaborates with interdisciplinary teams to coordinate resident assessments, develop care plans, and contribute to excellent patient care. Requirements

• Active Registered Nurse (RN) license.

Responsibilities

• Lead and coordinate the MDS assessment process, ensuring accuracy, completeness, and compliance with regulatory guidelines.
• Collaborate with nursing staff, therapists, social workers, and physicians to gather resident information and facilitate care planning.
• Monitor MDS data and reports to identify trends and areas for improvement in patient care.
• Ensure timely submission of MDS documentation to meet state and federal requirements.
• Provide education and training to staff regarding MDS processes and documentation standards.
• Participate in quality assurance and performance improvement initiatives related to MDS and patient care.
