About The Position

The MDS Assistant supports the comprehensive assessment and care-planning process for residents. The role assists with collecting, organizing, and entering clinical and functional data to ensure accurate Minimum Data Set (MDS) documentation. Working closely with nursing staff, therapists, and other healthcare professionals, the assistant gathers information that informs resident care plans and supports regulatory compliance. The position maintains accurate records and facilitates communication among care team members, contributing to resident-centered care, quality outcomes, and the facility’s compliance with state and federal guidelines.

Requirements

Current and valid Registered Nurse (RN) license in the state of Indiana.
Basic knowledge of healthcare terminology and clinical documentation.
Proficiency using computer systems and electronic health records.
Strong attention to detail and organizational skills.
Ability to communicate effectively with multidisciplinary teams.

Nice To Haves

Experience working in a long-term care or healthcare setting.
Familiarity with MDS assessment processes and regulatory requirements.
Completion of relevant healthcare or administrative training programs.
Demonstrated ability to manage multiple tasks and prioritize workload.

Responsibilities

Assist in collecting and verifying resident data for MDS assessments.
Collaborate with nursing and therapy staff to ensure timely and accurate documentation.
Enter and update resident information in electronic health record systems.
Support the MDS Coordinator in preparing reports and submitting data to regulatory bodies.
Maintain confidentiality and comply with healthcare regulations and facility policies.
