The Maternal Child Health Nurse manages a caseload of antenatal, postpartum, newborn, and pediatric home care patients. The nurse completes the nursing process—assessment, diagnosis, planning, implementation, and evaluation—using a family-centered and developmentally appropriate approach. The role collaborates with interdisciplinary team members, health care providers, and community services to meet patient needs and use services effectively, and provides clinical oversight of LPNs and LNAs in the home as needed. The nurse develops a plan of care under the direction of the patient’s physician and in collaboration with the patient and their family or primary caregivers. The nurse periodically reviews and recertifies the plan as appropriate, initiates referrals for interdisciplinary services, and communicates with other disciplines in a timely manner. The role includes ongoing assessment of patients’ clinical, functional, and mental status; reporting significant changes to the physician and clinical manager as appropriate; following agency policy for mandatory reporting; and providing relevant nursing interventions and education. The nurse reinforces education provided by other health care providers and may be assigned specialized procedures, work with patients in other programs such as Home Health and Hospice, or precept students and new staff. Other related duties may be assigned to meet organizational needs. This is a per diem, day-shift position serving the Upper Valley surrounding area, onsite. Dartmouth Health Home Care provides nursing, rehabilitation, hospice, and personal care services across more than 70 towns.
