About The Position

The Market Physician Executive (MPE) is a mission-driven physician leader responsible for leading Monogram Health’s in-home, multispecialty polychronic care model in an assigned market. Each market includes 5–10 territories supported by advanced practice providers, registered nurses, licensed clinical social workers, and pharmacists. The MPE collaborates with Monogram Health’s Multi-Specialty Platform and employed specialists to deliver in-home specialty care, using evidence-based pathways and a risk-based care model to improve patient experience, population health outcomes, provider satisfaction, and costs. Reporting to the Regional Vice President, the MPE contributes to the development and oversight of clinical strategies, policies, programs, processes, protocols, guidelines, and operations. The role oversees daily clinical and business operations, including direct patient care, care management, social work, and pharmacy services. The MPE reviews and approves care plans, directs patient treatment plans, and collaborates with community physicians, facilities, and partners in peer-to-peer consultations and patient-care decisions. The position is remote within the designated market, with occasional in-home patient visits and domestic travel.
