Monogram Health•Columbia, SC
•$260,000 - $340,000•Remote

About The Position

The Market Physician Executive (MPE) Float is a mission-driven physician leader responsible for leading Monogram Health’s in-home, multispecialty care model in an assigned market. Markets comprise 5–10 territories supported by advanced practice providers, registered nurses, licensed clinical social workers, and pharmacists. The MPE Float covers pod MPEs as needed, serves as the pod’s primary physician, and may eventually move into a permanent MPE role. The position collaborates with Monogram Health’s multispecialty platform and employed specialists to deliver in-home specialty care, using a risk-based model and proprietary AI algorithms to support appropriate care. The role aims to improve patient experience, population health outcomes, provider satisfaction, and costs through disease detection, evidence-based pathways, and treatment. Reporting to the Regional Vice President, the MPE Float contributes to developing and overseeing clinical strategies, policies, programs, processes, protocols, guidelines, and operations that improve patient outcomes. The role oversees daily clinical and business operations, including direct patient care, care management, social work, and pharmacy services. It supports advanced practice providers by reviewing and approving care plans and directing treatment plans, and collaborates with community physicians, facilities, and partners on peer-to-peer consultations and patient care decisions. The position is remote within the designated market, with occasional in-home patient visits and domestic travel. It participates in a variable on-call rotation, including at least seven days on call once per quarter, and provides coverage for other MPEs during PTO or vacancies as needed.
