About The Position

The Market Physician Executive (MPE) is a mission-driven physician leader responsible for leading Monogram Health’s in-home, multispecialty polychronic care model in an assigned market. Each market includes 5–10 territories staffed by advanced practice providers, registered nurses, licensed clinical social workers, and pharmacists. The MPE works with Monogram Health’s Multi-Specialty Platform to coordinate in-home specialty care and applies the company’s risk-based model and evidence-based clinical pathways to improve patient experience, population health outcomes, provider satisfaction, and cost of care. Reporting to the Regional Vice President, the MPE contributes to clinical strategy, policies, programs, processes, protocols, guidelines, and operations. The role oversees daily clinical and business operations, including direct patient care and care management services. The MPE supports advanced practice providers by reviewing and approving care plans and directing treatment plans, and collaborates with community physicians, facilities, and partners on clinical decisions. The position is remote within the designated market, with occasional in-home patient visits and domestic travel. The MPE participates in an on-call rotation, including a minimum of seven days on call at least once per quarter, and may provide coverage for other MPEs as needed.
