About The Position

The Manager - Integrated Case Management is part of a patient-centered care team, collaborating to assess, plan, and coordinate comprehensive healthcare solutions. The role addresses complex patient needs, optimizes health outcomes, and advocates for individuals and families across the care continuum. The position coordinates care across ambulatory, inpatient, and health plan settings and evaluates care plans and outcomes to promote patient safety, quality, and cost-effective care. It uses preventive service guidelines, clinical practice guidelines, and case management principles to guide care. The role is based with Henry Ford Health in Jackson, Michigan, serving patients across mid-Michigan.

Requirements

• Bachelor's degree in nursing or a related professional field, such as social work, counseling, or health education, or a Master of Social Work degree.
• Minimum of three years of clinical experience.
• Registered Nurse (RN) or Licensed Master Social Worker (LMSW) with a valid, unrestricted State of Michigan license.
• Excellent verbal communication, written documentation, and interpersonal skills.
• Strong problem-solving, analytical, and decision-making abilities.

Nice To Haves

• Certification in Case Management (CCM) by the Commission for Case Management Certification (CCMC).
• Three or more years of oncology experience for Henry Ford Cancer Institute roles.

Responsibilities

• Collaborate with the care team to assess, plan, and facilitate comprehensive healthcare solutions for patients with complex needs.
• Coordinate care across ambulatory, inpatient, and health plan settings to ensure seamless transitions and optimal outcomes.
• Advocate for patients and families by connecting them with community resources and addressing social determinants of health.
• Evaluate care plans and outcomes to promote patient safety, quality of care, and cost-effective solutions.
• Utilize preventive service guidelines, clinical practice guidelines, and case management principles to guide patient care.
