About The Position

Alvita Care is seeking experienced Licensed Practical Nurses to provide care for clients throughout Westchester County. Cases include private-duty, one-on-one care in clients’ homes and facility-based care. Shifts are 8 to 12 hours, with day and night shifts available. Nurses may work full-time or around their current work schedule. Cases are available only after successful completion of the agency’s onboarding process, and applicants undergo a phone screening to assess compatibility and availability.

Requirements

NYS LPN license.
BLS certification.
Minimum of one year of demonstrated experience as an RN, as stated in the posting.
Ability to put patients at ease and explain physicians’ instructions.
Ability to multitask and demonstrate excellent organizational skills.
Knowledge of medical terminology.
Ability to work as part of a team and independently.
Valid driver’s license or NYS Non-Driver photo ID card.

Responsibilities

Assess client needs and develop a plan of care.
Consult with management on complex or unique cases.
Develop and provide a plan of care with support from clinical, medical, and other appropriate multidisciplinary team members; coordinate, counsel, teach, and supervise services to meet client and patient needs.
Monitor and assess patients’ and clients’ conditions and the effects of their care plans.
Oversee and supervise care provided by Licensed Practical Nurses and paraprofessionals.
Manage, plan, coordinate, and carry out patients’ safe discharge from service.
Ensure physician orders, discharge summaries, and communications with team members, patients, and family members comply with company and regulatory requirements.
Communicate and consult with patients’ physicians as needed and manage and coordinate assigned caseloads.
Assist patients with activities of daily living.
Document the content and process of services provided in the internal system.
Perform other related duties and participate in special projects as assigned.
