About The Position
Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services. The Home Health Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) works under the supervision of a Registered Nurse to provide skilled nursing care to patients in their homes. The role includes patient and caregiver education, monitoring patient health, medication review, and performing ordered procedures. The position uses a combination of per-visit-point and hourly compensation. Annual total cash target pay is $59,498–$89,247, with a per-visit-point rate of $34.33–$51.49 and an hourly rate of $28.60–$42.91. The annual total cash range assumes full-time employment at 40 hours per week and full productivity; compensation varies based on factors including local labor markets, education, experience, productivity, and performance. Requirements
- At least 1 year of LPN or LVN experience.
- Current LPN license specific to the state(s) assigned for work.
- Current CPR certification.
- Valid driver's license, reliable transportation, and liability insurance.
- Pass a drug test before beginning employment.
Responsibilities
- Verify the plan of care with the RN before each visit.
- Collaborate frequently with the physician and RN to ensure patient needs are met, and notify them as soon as possible when a patient's condition changes.
- Teach patients and caregivers self-care techniques.
- Check vital signs.
- Review medications and perform medication reconciliation at each visit.
- Perform specialized procedures ordered by the physician, such as Foley catheter changes.
- Prepare equipment and materials for treatments using aseptic technique.
- Complete documentation accurately and on time.
