The LPN will assess patient and family needs, report new or pertinent findings to the RN and MD, and follow the prescribed plan of care as completed by the RN. The LPN will demonstrate the skills and judgments to implement the medical plan of care, nursing interventions, and procedures as necessary for patient care, within the guidelines of their licensure. They will evaluate and reassess identified problems, care provided, and patient's response on a continuing basis. The LPN will identify ancillary services needed by patients and families and report findings to the RN. They will collaborate and communicate on a regular basis with the Team Leader, Office Nurse, RN, and members of the inter-disciplinary team as needed. The LPN will assist patients and families in accessing appropriate community resources and record narrative documentation to reflect skilled care, ensuring documentation is clear, concise, and addresses specifics of teaching, direct care, and response of patient/caregiver. All other duties as assigned. The LPN will consistently provide service excellence to all patients, family members, visitors, volunteers, and co-workers in a manner that reflects VNA’s commitment to CARES: compassion, accountability, respect, excellence, and service.
