The LPN will be responsible for assessing patient and family needs, reporting findings to the RN and MD, and following the prescribed plan of care. This role requires the ability to implement the medical plan of care and nursing interventions within the scope of licensure. The LPN will also evaluate patient responses, identify ancillary services, and collaborate with the healthcare team. Accurate and clear documentation of skilled care, teaching, and patient response is essential. The position emphasizes providing service excellence in line with VNA's commitment to CARES (compassion, accountability, respect, excellence, and service).
