About The Position

The LPN will be responsible for assessing patient and family needs, reporting findings to the RN and MD, and following the prescribed plan of care. This role requires the ability to implement the medical plan of care and nursing interventions within the scope of licensure. The LPN will also evaluate patient responses, identify ancillary services, and collaborate with the healthcare team. Accurate and clear documentation of skilled care, teaching, and patient response is essential. The position emphasizes providing service excellence in line with VNA's commitment to CARES (compassion, accountability, respect, excellence, and service).

Requirements

  • Graduate of an approved LPN school of nursing.
  • Current LPN license in MA.
  • CPR certification required.
  • Minimum one (1) year recent experience in an acute care facility.

Responsibilities

  • Assesses patient and family needs.
  • Reports new or pertinent findings to RN and MD.
  • Follows prescribed plan of care as completed by the RN.
  • Implements the medical plan of care, nursing interventions, and procedures.
  • Evaluates and reassesses identified problems, care provided, and patient's response.
  • Identifies ancillary services needed by patients and families and reports findings to RN.
  • Collaborates and communicates on a regular basis with the Team Leader, Office Nurse, RN, and members of the inter-disciplinary team.
  • Assists patients and families in accessing appropriate community resources.
  • Records narrative documentation to reflect skilled care, teaching, direct care, and patient/caregiver response.
  • Performs other duties as assigned.
  • Provides service excellence to all patients, family members, visitors, volunteers, and co-workers.
