About The Position

The Physical Therapist will maintain annual HHRG values at the National Standard as appropriate, and utilize HHRG values to establish visit frequency and appropriate interdisciplinary utilization. The role involves ensuring annual outcomes meet National Benchmarks, referring to community resources as appropriate, and evaluating the clinical needs of the patient. The therapist will initiate, develop, implement, and revise the patient's plan of care, addressing all problems and documenting accordingly. Participation in inter-disciplinary patient case conferences and evaluation of the plan of care's effectiveness are key responsibilities. The role includes instructing and providing documented supervision of the Home Health Aide plan of care every 14 days, and notifying MD/other involved care providers of significant changes in the patient’s condition. Timely completion of admission and other paperwork, and clear, concise narrative documentation reflecting skilled care, teaching, direct care, and patient/caregiver response are essential. The therapist must obtain and clarify valid MD orders for all care delivered, and appropriately plan and coordinate discharges, informing the patient/MD and recording the disposition in the discharge summary. The position requires consistent service excellence to all patients, family members, visitors, volunteers, and co-workers in a manner that reflects VNA’s commitment to CARES: compassion, accountability, respect, excellence, and service.

Requirements

  • Bachelor of Science Degree or Master's Degree in Physical Therapy from an institution accredited by the Counsel on Medical Education of the American Medical Association and by the American Physical Therapy Association.
  • Current PT license in Mass.
  • One (1) year clinical experience.
  • CPR certification required.

Responsibilities

  • Evaluate the clinical needs of the patient; initiate, develop, implement and make necessary revisions to the patient's plan of care; address all problems in the plan of care and document accordingly.
  • Participate in inter-disciplinary patient case conferences.
  • Evaluate effectiveness of established plan of care.
  • Instruct and provide documented supervision of the Home Health Aide plan of care every 14 days.
  • Notify MD/other involved care providers of significant changes in the patient’s condition.
  • Complete admission and other paperwork on a timely basis.
  • Record narrative documentation to reflect skilled care. Documentation is clear, concise and addresses specifics of teaching, direct care and response of patient / caregiver.
  • Obtain and clarify valid MD orders for all care delivered.
  • Plan and coordinate discharges appropriately and inform patient/MD and record disposition of patient in the discharge summary.
  • Complete all appropriate discharge paperwork.
