The Physical Therapist will evaluate the clinical needs of the patient, initiate, develop, implement, and make necessary revisions to the patient's plan of care. This role addresses all problems in the plan of care and documents accordingly. The Physical Therapist will participate in inter-disciplinary patient case conferences and evaluate the effectiveness of the established plan of care. They will instruct and provide documented supervision of the Home Health Aide plan of care every 14 days. Additionally, the Physical Therapist will notify the MD/other involved care providers of significant changes in the patient’s condition and complete admission and other paperwork on a timely basis. They will record narrative documentation to reflect skilled care, ensuring it is clear, concise, and addresses specifics of teaching, direct care, and patient/caregiver response. The role involves obtaining and clarifying valid MD orders for all care delivered, planning and coordinating discharges appropriately, and informing the patient/MD and recording the disposition of the patient in the discharge summary. The Physical Therapist will also complete all appropriate discharge paperwork and consistently provide service excellence to all patients, family members, visitors, volunteers, and co-workers in a manner that reflects VNA’s commitment to CARES: compassion, accountability, respect, excellence, and service. The role also involves maintaining annual HHRG values at the National Standard as appropriate and utilizing HHRG values to establish visit frequency and appropriate interdisciplinary utilization, ensuring annual outcomes meet National Benchmarks and referring to community resources as appropriate.
