About The Position

Great Lakes Management is hiring a Licensed Practical Nurse (LPN) to support residents in an assisted living community at Amira Choice Arvado in Colorado. The LPN helps maintain a safe, nurturing environment and enhance residents’ quality of life. The role is scheduled three days per week, including rotating weekends, and pays $32–$38 per hour. A current Colorado LPN license is required.

Requirements

Current LPN license or certification in the State of Colorado, as required by state regulations.
Good organizational skills and the ability to self-start and follow through on initiatives with minimal supervision.
A caring disposition and passion for caring for older adults.

Nice To Haves

At least one year of experience in home health, assisted living, or long-term care is preferred.

Responsibilities

Ensure residents continue to meet admission criteria, monitor their status, and recommend alternative placement when necessary.
Ensure residents have current care plans in accordance with state regulations.
Monitor and maintain resident charts and clinical progress notes.
Communicate changes in residents’ needs or conditions to physicians, staff, and families.
Coordinate resident services with other service providers.
Oversee medications and renewals, including necessary follow-up with physicians about medications or medical conditions.
Ensure medication administration procedures are followed and discuss needed corrections or errors with staff.
Record information that staff need to know in residents’ logs.
Conduct alternating supervisory visits in coordination with the RN.
Work collaboratively with all members of residents’ care teams.
Supervise medication practices and medical equipment used in the building.
Ensure staff understand and follow resident care procedures.
Share resident and family concerns with appropriate staff.
Work with the manager to ensure emergency procedures are in place.
