About The Position

Great Lakes Management is hiring an LPN to support residents in an assisted living community. The LPN helps maintain a safe, nurturing environment and enhance residents’ quality of life by coordinating care, monitoring health and clinical records, overseeing medication practices, and working with residents, families, staff, physicians, and other care providers. The position is located at Amira Choice in Arvada, Colorado. The schedule is two days per week, with potential rotating weekends. Pay is $32–$38 per hour.

Requirements

Maintain a current LPN license in the state of Colorado, in accordance with state regulations.
A caring disposition and passion for caring for older adults.
Good organizational skills and the ability to self-start and follow through with initiatives with minimal supervision.

Nice To Haves

At least one year of experience in home health, assisted living, or long-term care is preferred.

Responsibilities

Ensure residents continue to meet admission criteria, monitor their status, and recommend alternative placement when necessary.
Ensure residents have current care plans in accordance with state regulations.
Monitor and maintain resident charts and clinical progress notes.
Communicate changes in residents’ needs or conditions to physicians, staff, and family members.
Coordinate resident services with other service providers.
Oversee medications and renewals, including necessary follow-up with physicians about medications or medical conditions.
Ensure medication administration procedures are followed and discuss needed corrections or errors with staff.
Record information staff need to know in residents’ logs.
Provide alternating supervisory visits in coordination with the RN.
Work as a team member with all members of residents’ care teams.
Supervise medication practices and medical equipment used in the building.
Ensure staff understand and follow resident care procedures.
Share resident and family concerns with appropriate staff when appropriate.
Work with the manager to ensure emergency procedures are in place.
