About The Position

Great Lakes Management is hiring a Licensed Practical Nurse (LPN) to support residents in an assisted living community. The LPN helps enhance residents’ quality of life by contributing to a safe, nurturing environment and coordinating resident care. The position is located at Amira Choice Arvada, Colorado, and is scheduled for three days per week with rotating weekends. The listed pay range is $32–$38 per hour.

Requirements

Maintain a current LPN license in Colorado in accordance with state regulations.
A caring nature and passion for caring for older adults.
Good organizational skills and the ability to self-start and follow through on initiatives with minimal supervision.

Nice To Haves

At least one year of experience in home health, assisted living, or long-term care is preferred.

Responsibilities

Ensure residents continue to meet admission criteria, monitor their needs, and recommend alternative placement when necessary.
Ensure residents have current care plans in accordance with state regulations.
Monitor and maintain resident charts and clinical progress notes.
Communicate changes in residents’ needs or conditions to physicians, staff, and families.
Coordinate resident services with other service providers.
Oversee medications and renewals, including follow-up with physicians about medications or medical conditions as needed.
Ensure medication-administration procedures are followed and discuss needed corrections or errors with staff.
Record information staff need to know in residents’ logs.
Provide alternating supervisory visits in coordination with the RN.
Work as a team member with the residents’ care team.
Supervise medication practices and medical equipment used in the building.
Ensure staff understand and follow resident-care procedures.
Share resident and family concerns with relevant staff when appropriate.
Work with the manager to ensure emergency procedures are in place.
