About The Position

Prosper at Wickliffe is seeking an LPN for its assisted living community. The LPN works with the interdisciplinary team, residents, and families to assess needs, develop Individual Service Plans (ISPs), and coordinate the services identified in each plan. Working with the Resident Services Director and caregivers, the LPN helps provide optimum care for each resident and supervises staff, including taking disciplinary action when needed. The role may also involve performing technical nursing skills.

The LPN supports and assesses caregivers while assisting with direct resident care, serves as a liaison among staff, administration, families, service providers, and physicians, and helps coordinate educational programs for personnel. In collaboration with the Resident Services Director, the LPN directs and evaluates resident care, implements ISPs for new residents, addresses safety hazards, and completes reports and assignments accurately and on schedule.

Responsibilities

• Assist staff with direct resident care, serve as a role model, and assess caregivers’ skills and working conditions.
• Act as a liaison between staff and administration, families, service providers, and physicians to promote optimal resident care.
• Work with the Resident Services Director to develop and coordinate community educational programs, including personnel orientation and in-service training.
• Work with the Resident Services Director to direct, evaluate, and supervise resident care and initiate corrective action as necessary.
• Review newly admitted residents and implement an Individual Service Plan for each resident with the interdepartmental team.
• Identify safety hazards and initiate corrective action as necessary.
• Complete reports and assignments accurately and adhere to established time schedules.
• Provide general staff supervision and take disciplinary action when needed.
• Perform technical nursing skills as needed.
