About The Position

Byron Wellness Community is seeking a Licensed Practical Nurse to provide direct nursing care and help coordinate and support resident care throughout the community. Nursing at Byron emphasizes relationships, advocacy, dignity, purpose, and quality of life, with a resident-centered approach that recognizes each person beyond their diagnosis. The LPN will work with residents, families, nursing assistants, medication aides, physicians, pharmacies, therapists, and interdisciplinary teams. Full-time work is available on the second shift, with every other weekend and rotating holidays required.

Requirements

Possess a nursing degree from an accredited college or university, or graduate from an approved RN/LPN program.
Hold an active license.

Experience working with individuals living with dementia, behavioral health challenges, developmental disabilities, traumatic brain injuries, or other specialized populations.

Responsibilities

Provide direct nursing care and treatment services.
Administer medications and monitor their effectiveness.
Assess resident needs and report changes in condition.
Supervise and support nursing assistants and medication aides.
Coordinate care with physicians, pharmacies, therapists, and interdisciplinary teams.
Participate in admissions, transfers, and discharges.
Update and implement resident care plans.
Complete required documentation and clinical records.
Assist with quality assurance and regulatory compliance.
Support residents and families through challenging situations.
Help maintain a safe, respectful, resident-centered environment.
Advocate for residents and help ensure they receive appropriate care, dignity, and quality of life.
