About The Position

As the Weekend Special LPN, you will perform the functions of a Licensed Practical Nurse in providing patient care to hospital patients ages 18 through geriatric. The role includes hands-on nursing care, medication administration, assessment collection, patient and family education, and accurate clinical documentation. It also involves coordinating with nursing and interdisciplinary teams to support quality improvement, wound and ostomy care, dietary oversight, and patient-centered care.

Requirements

• Current, unrestricted LPN license in the state of practice.
• Current ACLS certification or ability to complete it within 90 days of hire.

Nice To Haves

• Current CPR certification or ability to complete it within 90 days of hire.
• ICU experience.
• CCRN or PCCN certification.

Responsibilities

• Deliver and document patient-centered care by performing hands-on nursing tasks, administering medications, collecting assessments, and educating patients and families in alignment with hospital policies and the Patient’s Bill of Rights.
• Coordinate with the DON, therapists, and nursing staff on departmental changes and performance improvement initiatives, and report changes in patient condition in a timely manner.
• Demonstrate proficiency in central line management, ventilator settings, and cardiac monitoring.
• Supervise patient meal accuracy and portion sizes, coordinating timely corrections with the Registered Dietician.
• Educate and support patients and families to promote patient independence, goal achievement, and preparation for decision-making.
• Provide direct care to wound and ostomy patients and collaborate with interdisciplinary teams to manage and monitor their care.
• Lead staff education and quality improvement efforts related to wound care.
• Collaborate with medical staff and coding teams and use performance analysis to ensure complete and accurate clinical documentation.
