About The Position

The Neighborhood at Paducah is seeking a Wellness Nurse (LPN/Nurse MedTech) to join their team. This role is responsible for monitoring the health and wellbeing of residents, providing leadership support, and assisting with resident care and services. Key responsibilities include medication administration, communication with resident support services and families, and maintaining clinical quality assurance. The Wellness Nurse also leads, demonstrates, and supports all elements of wellness that are core components of Phoenix programs and services.

Requirements

Must be a Licensed Practical Nurse or Registered Nurse.
Ability to work weekends, evenings and flexible hours, available to our customers at peak service delivery days and times.

Nice To Haves

A minimum of one year of experience working with elders or disabled individuals.
Demonstrated Leadership capabilities.
Knowledge and experience in Assisted Living industry and Dementia care or Skilled Nursing.

Responsibilities

Monitoring the health and wellbeing of residents.
Providing leadership support and assistance with resident care and services.
Medication administration.
Communication with resident support services and families.
Maintaining clinical quality assurance.
Leading, demonstrating, and supporting all elements of wellness.
Identifying on-going needs and services of residents through the assessment / Personalized Service Plan (PSP) process.
Ensuring proper follow through and documentation for residents with a change in clinical care needs.
Assisting the Wellness Director with completing the clinical sections of monthly wellness visits and PSP.
Informing the Wellness Director and/or Executive Director of any changes in medication or service level.
Assisting with obtaining weights and vital signs monthly for each resident.
Assisting with direct resident care as needed.
