LPN - Schedule Coordinator Home Health - Full-time
About The Position
Enhabit Home Health & Hospice is seeking a licensed practical or vocational nurse to coordinate patient scheduling for field clinicians. The role supports care coordination and continuity by communicating with field staff, patients, physicians, referral sources, caregivers, and other service providers. It also involves managing the on-call notebook and hospitalization logs to improve communication among stakeholders. In-office attendance is essential except for designated full-time remote positions; occasional remote work may be permitted for temporary personal needs or circumstances beyond the company’s control.
Requirements
- High school diploma or equivalent.
- Licensed practical or vocational nurse.
- Maintain a valid clinical license in accordance with state requirements and applicable board rules.
- At least one year of clinical experience in a healthcare setting, or at least one year of home health, hospice, or pediatric experience within the last 24 months with demonstrated understanding of home-care staffing and scheduling requirements.
- Maintain CPR certification as required by the position.
- Maintain a valid driver license in the state of current residence.
- Maintain automobile liability insurance as required by law.
- Basic demonstrated technology skills, including operation of a mobile device; intermediate technology skills are also specified, especially Microsoft Word and Excel.
- Dependable transportation in good working condition and ability to drive safely in all types of weather.
- Oral and written communication skills, fluency in English, and active listening.
- In-office attendance is essential except for designated full-time remote positions.
Nice To Haves
- Previous experience using Homecare Homebase (HCHB).
- Bilingual English/Spanish ability to facilitate communication with patients and caregivers and support positive patient outcomes.
