About The Position

Consumer Direct Care Network is seeking a compassionate and skilled Home Care Nurse to provide personalized skilled nursing care to members in their homes. The nurse supports individuals with disabilities and older adults through home and community-based services, helping them remain in their homes and communities. This role includes assessing members’ health, creating and implementing individualized care plans, administering prescribed medications and treatments, documenting care, educating members and families, and coordinating with other healthcare professionals. The position is listed as part-time, with expected hours of 10–40 per week, and pays $28.00–$38.00 per hour.

Requirements

Valid nursing license in Montana.
Excellent communication, documentation, and interpersonal skills.
Ability to work independently and prioritize tasks effectively.
Current driver’s license and auto insurance.
Current BLS certification.
Strong knowledge of human anatomy and physiology.
Competence in medication administration.
Ability to adhere to professional and industry standards of practice and care.

Responsibilities

Conduct comprehensive assessments of members’ health status.
Develop and implement individualized care plans.
Observe, identify, and document changes in clients’ physical, emotional, mental, or social condition; reactions to medications; and environmental safety factors.
Administer prescribed medications and treatments.
Maintain accurate and timely records while protecting the confidentiality of medical records.
Educate members and their families on healthcare procedures and disease management.
Collaborate with other healthcare professionals to coordinate effective care.
