About The Position

Albany Home Health and Hospice is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our On-Call/After Hours Team. This role provides skilled nursing care, after-hours triage support, and home visits for home health and hospice patients throughout the Albany and Linn-Benton area.

Requirements

• Current Oregon LPN License required.
• Graduate of an accredited nursing program.
• Current CPR certification.
• 1–2 years of acute care nursing experience.
• Valid driver's license, reliable transportation, and auto insurance.
• Strong communication, assessment, and independent decision-making skills.

Nice To Haves

• Home Health or Hospice experience preferred.

Responsibilities

• Respond to after-hours patient calls and triage needs.
• Provide skilled nursing visits in patient homes.
• Complete admissions and follow-up visits as needed.
• Administer medications and treatments as ordered.
• Assess patient conditions and communicate changes to providers.
• Educate patients and caregivers on care plans and symptom management.
• Maintain accurate and timely documentation.
• Collaborate with physicians, case managers, and interdisciplinary teams.
