About The Position
Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services. The RN On-Call After Hours Hospice nurse provides after-hours clinical support, triages patient needs, and coordinates care to support continuity and quality outcomes. The role includes remote and field-based nursing support, patient assessments, and collaboration with interdisciplinary teams to address urgent and emergent needs. The coverage territory is Kent and Sussex County, Delaware. The schedule is Monday through Thursday, 5 p.m. to 8 a.m.
Requirements
- Current RN license specific to the state(s) assigned.
- At least 1 year of nursing experience; hospice, home health, or acute care experience is preferred.
- Current CPR certification.
- Valid driver's license, reliable transportation, and liability insurance.

Responsibilities
- Provide on-call nursing support, triage patient needs, and determine appropriate interventions.
- Assess patient conditions by phone and/or in-person visits and escalate care as needed.
- Coordinate with physicians, care teams, and caregivers to ensure timely and effective care delivery.
- Educate patients and caregivers about symptom management and care plans.
- Maintain accurate and timely documentation of patient interactions and clinical decisions.
