About The Position
Licensed Practical Nurse position focused on providing direct patient care in a home health setting. The role involves routine visits to patients, management of wound care, and coordination with the care team using HomeCare HomeBase as the electronic health record system.
Requirements
- Basic Life Support (BLS) certification from the American Heart Association (AHA).
- Valid driver's license and insurance.
- Experience with HomeCare HomeBase (HCHB) electronic health records.
- Experience in wound care, wound vacs, and ostomies.
Responsibilities
- Provide wound care, including management of wound vacs and ostomies.
- Caring for patients with cardiac conditions such as CHF and pneumonia, and those undergoing IV antibiotics therapy with PICC line care.
- Perform routine visits, handling up to 7 patient visits a day using a point system.
