About The Position

Phoenix Home Health is seeking an LPN to provide home health nursing care in Fordland, Seymour, Mansfield, Ava, Hartville, Mountain Grove, and surrounding areas. The nurse serves as a link between patients and medical providers, performs physical assessments, and carries out individualized care plans based on physician orders and assessment findings. The role involves collaborating with nurses, physical therapists, speech therapists, and occupational therapists. Charting is completed on-site with support from Roger’s AI-powered system.

This is a full-time, daytime position, Monday through Friday, with a weekend rotation every four to six weeks. The stated schedule is 8 a.m. to 5 p.m. The pay range is $50,000–$80,000.

Requirements

• Missouri or Compact Registered Nursing or Licensed Practical Nursing license.
• Strong organizational and self-management skills.
• Strong and compassionate customer service skills.
• Valid driver’s license.

Responsibilities

• Assist in reviewing and revising the plan of care.
• Provide nursing care in accordance with the physician’s plan of treatment and the individual plan of care, as authorized by the client’s payer source.
• Observe and record the client’s response to treatment and report changes in the client’s condition to appropriate personnel and/or the physician.
• Provide ongoing instruction to the client or family about the plan of treatment.
• Participate in education programs, quality assessment activities, nurses’ meetings, and other staff meetings as required.
