LNA Hospice Aide - Concord
About The Position
Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services. The LNA Hospice Aide provides compassionate, hands-on care to hospice patients and supports their families during end-of-life care. The role assists patients with daily living activities while promoting comfort, dignity, and emotional well-being. Working with the hospice care team, the aide provides respectful, individualized care in accordance with each patient's care plan. The service area includes Concord and surrounding areas. The position is described as per diem. Pay ranges from $17.55 to $26.35 per hour based on full-time employment; actual pay depends on factors including local labor markets, education, work experience, and certifications. Benefits and incentives are subject to eligibility requirements. Candidates must pass a drug test before beginning employment.
Requirements
- Current state certification or registration as a Certified Nursing Assistant (CNA), in accordance with state requirements.
- Current CPR certification.
- Valid driver's license, reliable transportation, and liability insurance.

- CNA experience within the last two years.
Responsibilities
- Provide personal care services to patients.
- Apply safety principles and proper body mechanics when assisting with ambulation, transfers, range-of-motion activities, positioning, and household chores.
- Plan and prepare nutritious meals and assist with feeding as assigned.
- Perform homemaking and other environmental services as assigned.
- Observe, report, and document changes in patient status, and notify the primary nurse when unable to perform a specific task.
- Follow written personal-care instructions prepared by a Registered Nurse in accordance with the plan of care.
- Assist patients with prescribed exercises as instructed by a nurse or therapist.
- Perform and record simple urine tests for sugar, acetone, and albumin.
- Perform other duties as assigned.
