About The Position

The Choice Hospice Aide provides care to patients in their place of residence, including personal healthcare assistance and related services, and helps maintain a safe and clean environment. The aide works cooperatively with patients and families and shares observations and problems with the supervisor. Hospice Aides are a key part of the care team and help deliver the mission of hospice. The position requires weekly travel to Ada for supplies.

Requirements

• Current CNA/HHA and LTC certifications.
• Valid driver's license.
• Valid auto insurance.
• CPR certification.

• HomeCare HomeBase experience preferred.

Responsibilities

• Provide direct patient care under the direction of the RN and according to the Aid Plan of Care.
• Assist patients with safe transfers and ambulation in accordance with agency policy.
• Report changes and document pertinent information and care provided to support continuity of care.
• Practice accepted infection control principles and provide a clean, safe, and comfortable environment.
• Promote positive, supportive, and respectful communication with patients, families, and other employees.
• Respect patients’ privacy and property.
• Assist patients with proper nutrition and adequate fluid intake.
• Contribute to agency management and efficient operations, demonstrating effective time management.
• Perform treatments and procedures according to agency policy.
• Promote the agency philosophy and administrative policies to ensure quality of care.
• Travel to Ada weekly for supplies.
