About The Position
Enhabit Home Health & Hospice is hiring an LMSW to administer social services to patients requiring intermittent professional services and to teach patients, families, and other members of the health care team. Services must be provided in accordance with physician orders and the established plan of care. The role requires a relevant social work degree, state certification or licensure, health care social work experience, technology skills, and the ability to meet the stated driving and CPR requirements. Medicare home health or hospice experience is preferred.
Requirements
- Must have a master’s degree from a school of social work accredited by the Council on Social Work Education.
- Must be certified or licensed in the state where they currently practice, subject to the state-specific qualifications listed in the posting.
- Must have at least one year of social work experience in a health care setting.
- Must have basic demonstrated technology skills, including operation of a mobile device.
- Must possess a valid state driver license.
- Must maintain automobile liability insurance as required by law.
- Must maintain dependable transportation in good working condition.
- Must be able to safely drive an automobile in all types of weather conditions.
- Must possess CPR certification.
