About The Position

This full-time, day-shift position provides social work services to hospice patients and their families in accordance with the plan of care. Compensation is $60,000–$65,000 and may vary by location, job-related knowledge, skills and experience.

Requirements

• Master's or doctorate degree from a school of social work accredited by the Council on Social Work Education.
• One year of social work experience in a healthcare setting.
• Meet state and federal requirements for the position.
• Current CPR certification.

Responsibilities

• Assist patients and families with health-related financial, social and emotional concerns, and help them develop effective coping skills.
• Provide social work services in accordance with the plan of care.
• Communicate changes in a patient's clinical status promptly to other healthcare team members and care center office staff.
• Educate patients, family members and other team members about medical social work interventions and the treatment plan.
• Perform care planning, transfer and discharge functions.
• Identify risks or threats to a patient's health in their environment and promptly implement appropriate interventions to reduce risks.
• Communicate identified risks or threats to the physician and other healthcare team members to coordinate care effectively.
• Perform other duties as assigned.
