About The Position

Elara Caring is hiring a Licensed Vocational / Practical Nurse to provide compassionate, high-quality care in a home care setting. The nurse follows individualized care plans and physician orders, monitors patient progress, helps patients and caregivers learn self-care, and works with the RN Case Manager to update care plans as needs change. The role also includes documenting medication regimens, coordinating with physicians, patients, families, and the care team, and proactively supporting positive patient outcomes while helping prevent avoidable hospitalizations and emergency room visits.

Requirements

• Current state license as a Licensed Vocational / Practical Nurse.
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing.
• One year of experience in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver's license and maintain an acceptable driving record, or have reliable transportation to travel between patient homes and assigned work locations.

Responsibilities

• Follow each patient's individualized care plan and physician orders.
• Monitor patient progress and report changes or safety concerns to the care team.
• Help patients and caregivers learn self-care techniques to support their health.
• Work with the RN Case Manager to update care plans as patient needs change.
• Document medication regimens and update medication profiles according to agency guidelines.
• Communicate with physicians, patients, families, and the care team to ensure coordinated care.
• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.
