About The Position

As an On-Call Licensed Practical/Vocational Nurse with Affinity Hospice Care, you will help patients heal, stay safe, and thrive in the comfort of their own homes. The role supports patients and families during the hospice transition through compassionate, high-quality home hospice care in a supportive, family-oriented environment. Evening standby hours are 5 p.m. to 7 a.m., with a rate of $80 per evening. Weekend daytime standby hours are 7 a.m. to 5 p.m., with a rate of $120 per day. The position may convert to full-time based on performance if desired.

Requirements

• Graduate of an accredited practical or vocational nursing program.
• Active LPN or LVN license in California.
• At least 3 years of nursing experience.
• Current CPR certification with AHA.
• Valid driver’s license, reliable insured vehicle, and ability to travel to patient homes.
• Strong communication skills and a collaborative, patient-first mindset.

Nice To Haves

• Home health or medical-surgical background preferred.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team, including OT, PT, HHA, and other disciplines, and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely while adhering to aseptic technique.
• Perform other duties as required for patient services.
