About The Position

Elara Caring is seeking a compassionate Licensed Vocational / Practical Nurse to provide home care and support positive patient outcomes. The nurse follows individualized care plans and physician orders, monitors patients, supports patients and caregivers with self-care, and coordinates with the RN Case Manager and other members of the care team. The role includes documenting medication regimens, communicating with physicians, patients, and families, and providing proactive care to help prevent avoidable hospitalizations and emergency room visits.

Requirements

• Current state license as a Licensed Vocational / Practical Nurse.
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing.
• One year of experience in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Valid driver's license with an acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations.

• Experience in a hospice or home health environment.

Responsibilities

• Follow each patient's individualized care plan and physician orders.
• Monitor patient progress and report changes or safety concerns to the care team.
• Help patients and caregivers learn self-care techniques to support their health.
• Work with the RN Case Manager to update care plans as patient needs change.
• Document medication regimens and update medication profiles according to agency guidelines.
• Communicate with physicians, patients, families, and the care team to ensure coordinated care.
• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.
• Deliver compassionate, high-quality care that supports positive patient outcomes.
