About The Position

Elara Caring is hiring a PRN Licensed Vocational Nurse (LVN) for its Home Health team in Harker Heights, Texas. The nurse provides one-on-one skilled nursing care in patients’ homes, helping them recover, manage their health, and remain safely at home. The LVN works with an RN Case Manager and an interdisciplinary clinical team to deliver care based on individualized care plans and physician orders, educate patients and caregivers, monitor progress, and coordinate care. PRN visits vary according to census and coverage needs, and hours are not guaranteed. Applicants should share their availability with the team.

Requirements

Active Texas Licensed Vocational Nurse (LVN) license.
Graduation from an accredited nursing program.
At least one year of clinical nursing experience.
Consistent availability to support patient visits and communicate schedule with the team.
Valid driver’s license, reliable transportation, and current auto insurance.
Ability to travel throughout the assigned territory.
Ability to sit, stand, bend, lift, and move intermittently, and lift 50–100 lbs.

Responsibilities

Provide skilled nursing care in patients’ homes according to individualized care plans and physician orders.
Monitor patient progress and promptly report changes in condition or safety concerns to the RN Case Manager.
Educate patients and caregivers about medications, self-care, and ways to support recovery and independence.
Partner with the RN Case Manager and clinical team as patient needs evolve.
Complete accurate, timely documentation of visits, nursing interventions, and medication information.
Communicate with physicians, families, and caregivers to support coordinated care.
Help prevent avoidable hospitalizations through attentive care and early recognition of concerns.
