About The Position

The Licensed Practical/Vocational Nurse provides direct patient care under the supervision of a registered nurse. The role follows the plan of care, provides treatments, and collaborates with team members to support positive patient care outcomes. The position is with Nova Hospice, an independent operating subsidiary of The Pennant Group, Inc., a holding company whose subsidiaries provide healthcare services through home health and hospice agencies and senior living communities.

Requirements

• Complete a requisite practical (vocational) nursing program or be licensed in the state where the HHA is located.
• Hold a current license as a licensed practical nurse or licensed vocational nurse in the state.
• Comply with accepted professional standards and practice.
• Demonstrate good verbal and written communication and organization skills.
• Be a licensed driver with an automobile that is insured in accordance with state and organization requirements and is in good working order.
• Possess and maintain current CPR certification.

Nice To Haves

• Three years of nursing experience preferred.
• Community/home health or medical/surgical experience preferred.

Responsibilities

• Provide direct patient care as defined by the state’s Nurse Practice Act.
• Implement the plan of care initiated by the registered nurse.
• Provide accurate and timely documentation consistent with the plan of care.
• Collect assessment data and perform observations, reporting and collaborating with registered nurses and physicians.
• Participate in coordination of home health services and report identified needs for other disciplines—including HHA, OT, PT, MSW, ST, and dietitian—to the registered nurse and/or Assistant Director of Nursing or Director of Nursing.
• Use equipment and supplies effectively and efficiently.
• Participate in personal and professional growth and development.
• Perform other duties as assigned by the registered nurse.
