About The Position

The Licensed Practical/Vocational Nurse provides direct patient care under the supervision of a registered nurse. The role follows the established plan of care, provides treatments, documents care, and collaborates with team members to support positive patient outcomes.

Requirements

• Complete a requisite practical (vocational) nursing program or be licensed in the state where the HHA is located.
• Hold a current license as a licensed practical nurse or licensed vocational nurse in the state.
• Comply with accepted professional standards and practice.
• Demonstrate good verbal and written communication and organization skills.
• Be a licensed driver with an automobile insured in accordance with state and organization requirements and in good working order.
• Possess and maintain current CPR certification.

Nice To Haves

• Three years of nursing experience preferred.
• Community/home health or medical/surgical experience preferred.

Responsibilities

• Provide direct patient care as defined by the state Nurse Practice Act.
• Implement the plan of care initiated by the registered nurse.
• Document care accurately and promptly, consistent with the plan of care.
• Collect assessment data and observe patients, reporting findings to and collaborating with registered nurses and physicians.
• Participate in coordinating home health services by reporting identified needs for other disciplines, including HHA, OT, PT, MSW, ST, and dietician, to the registered nurse and/or Assistant Director of Nursing or Director of Nursing.
• Use equipment and supplies effectively and efficiently.
• Participate in personal and professional growth and development.
• Perform other duties as assigned by the registered nurse.
