About The Position

St. Hope Healthcare is seeking a Licensed Social Worker (LBSW or LMSW) for Bellaire Health Center. The role provides medical assessments, short-term intensive interventions, and consultation services, including medical case management and support for patients’ treatment adherence, care coordination, and transition from services.

Requirements

• Bachelor’s degree in social work from an accredited university or college.
• Licensed Social Worker in the State of Texas or a transferable license from another state.
• At least 2 years of paid experience in the case management field.
• Experience working in a primary care or family practice setting.
• Excellent written and verbal communication skills.
• Detail-oriented and organized.
• Self-motivated, with the ability to work independently.
• Strong professional ethics and boundaries.
• Ability to work with frequent interruptions and maintain emotional control under stress.
• Willingness to travel to Bellaire Health Center.

Nice To Haves

• Bilingual in Spanish and English.

Responsibilities

• Provide intensive medical case management.
• Conduct assessments and readiness assessments, and provide patient medication education to support successful adherence.
• Refer patients for medical appointments and follow up on their status.
• Provide resources and referrals.
• Monitor treatment compliance.
• Serve as a liaison and assist in coordination between medical staff and patients.
• Participate in discharge planning.
• Develop a service plan for each patient.
• Transition clients out of medical case management services when they have completed the service plan and achieved medical stabilization.
